Before designing, it is good practice to create the dataset for your project first.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 1 \u2013 Open the Questionnaire Design Screen<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">You can open the design screen in three ways:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Click the&nbsp;<strong>Design questionnaire<\/strong>&nbsp;icon in the Survey Navigator toolbar<\/li>\n\n\n\n<li>Select&nbsp;<strong>Design Questionnaire<\/strong>&nbsp;from the&nbsp;<strong>Design<\/strong>&nbsp;menu<\/li>\n\n\n\n<li>Press&nbsp;<strong>F8<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">The Questionnaire Design Screen opens.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><code>[SCREENSHOT: Questionnaire Design Screen]<\/code><\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Important:<\/strong>&nbsp;Only one person can design the same questionnaire at a time. Opening the same project in multiple sessions simultaneously can cause errors and data loss.<\/p>\n<\/blockquote>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">The three panes<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">The design screen consists of three panes:<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Left \u2013 Questionnaire Structure<\/strong>&nbsp;An overview of all forms in your questionnaire, displayed as a tree structure. Click any form name to select and edit it.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Middle \u2013 Form<\/strong>&nbsp;Settings and options for the selected form. The available tabs depend on the form type. You can rename a form here using the&nbsp;<strong>Rename<\/strong>&nbsp;button.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><strong>Right \u2013 Content<\/strong>&nbsp;The area where you write the question text. For question types with answer options, the lower half of this pane shows the answer list. HTML and JavaScript can also be inserted here if needed.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 2 \u2013 Add an introduction to the Start form<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Every questionnaire starts with a&nbsp;<strong>Start<\/strong>&nbsp;form, which is added automatically by Dub Quest. Select it in the Questionnaire Structure pane and type an introduction text in the Content pane.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><code>[SCREENSHOT: Start form with introduction text]<\/code><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 3 \u2013 Create your first question<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">In the&nbsp;<strong>Default routing<\/strong>&nbsp;field at the bottom of the middle pane, type the name of your first question. Then press&nbsp;<strong>Ctrl+Enter<\/strong>. A confirmation dialog appears asking whether you want to create a new form with that name. Click&nbsp;<strong>Yes<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><code>[SCREENSHOT: Confirmation dialog]<\/code><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Next, choose the form type. Select&nbsp;<strong>Question form<\/strong>, then choose a question type such as&nbsp;<strong>Multiple Response<\/strong>. Click&nbsp;<strong>OK<\/strong>. The new form appears in the Questionnaire Structure.<\/p>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p class=\"wp-block-paragraph\"><strong>Naming tip:<\/strong>&nbsp;Once you name a form, avoid renaming it afterwards. Other forms, expressions and conditions may reference that name, and changing it can break those references \u2014 similar to changing the URL of a webpage.<\/p>\n<\/blockquote>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 4 \u2013 Add question text and answer options<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Select your new form in the Questionnaire Structure. In the Content pane:<\/p>\n\n\n\n<ol class=\"wp-block-list\">\n<li>Type your question text in the upper area<\/li>\n\n\n\n<li>In the&nbsp;<strong>Answers<\/strong>&nbsp;section at the bottom, click&nbsp;<strong>New<\/strong>&nbsp;to add answer options<\/li>\n<\/ol>\n\n\n\n<p class=\"wp-block-paragraph\">The&nbsp;<strong>Add new item<\/strong>&nbsp;window appears. Enter the answer text in the&nbsp;<strong>Description<\/strong>&nbsp;field and click&nbsp;<strong>OK<\/strong>. Repeat for each answer option.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><code>[SCREENSHOT: Add new item window]<\/code>&nbsp;<code>[SCREENSHOT: Answer items in the Content pane]<\/code><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Use the&nbsp;<strong>Edit<\/strong>&nbsp;button to modify an existing answer, or&nbsp;<strong>Delete<\/strong>&nbsp;to remove one.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 5 \u2013 Add more questions<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Use the&nbsp;<strong>Default routing<\/strong>&nbsp;drop-down in the middle pane to route to the next question and create additional forms. Repeat Step 3 and Step 4 for each question.<\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 6 \u2013 Add an End form<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Every questionnaire must end with at least one&nbsp;<strong>End of questionnaire<\/strong>&nbsp;form. Create a new form and set its type to&nbsp;<strong>End of questionnaire<\/strong>&nbsp;in the Type drop-down in the middle pane.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><code>[SCREENSHOT: Design Screen with End form visible]<\/code><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<h3 class=\"wp-block-heading\">Step 7 \u2013 Check the integrity<\/h3>\n\n\n\n<p class=\"wp-block-paragraph\">Close the design screen with&nbsp;<strong>Alt+F4<\/strong>&nbsp;or the close button. The system will ask if you want to check the integrity of the questionnaire. Click&nbsp;<strong>Yes<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><code>[SCREENSHOT: Integrity check prompt]<\/code><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dub Quest checks the questionnaire and reports any errors. If everything is correct, a confirmation message appears. If there are errors, they are listed at the bottom of the screen with explanations.
